Personal health support available for HealthSelect participants

If you are enrolled in a HealthSelect of Texas® plan, you have extra support for managing various health conditions, including heart disease, diabetes, high blood pressure and asthma through the Blue Cross and Blue Shield of Texas care management program. One of the program’s Texas-based clinicians can also work with your providers to coordinate your treatment plan, arrange appointments and coordinate transportation to and from appointments.

The care management program is available for HealthSelect of Texas, Consumer Directed HealthSelectSM, HealthSelectSM Out-of-State and HealthSelectSM Secondary plan participants, at no out-of-pocket cost.

Here’s how it works: Isabella’s Journey
 
Isabella has Type-2 diabetes and high blood pressure. She’s 47 and going through a difficult divorce. She’s feeling stressed. With everything going on, she has lost track of what she needs to do to keep her blood sugar in check.

Isabella learns she can get help from the care management program offered through her HealthSelect of Texas medical plan. She calls the number on the back of her BCBSTX medical ID card, (800) 252-8039 (TTY:711), and asks to speak with a clinician.

Isabella explains her situation to the clinician, Anna.

After gathering a brief health history, Anna sees that Isabella hasn’t visited her endocrinologist in some time and is due for a check-up. Anna also reminds Isabella about her mental health benefits, including mental health Virtual Visits.

After speaking with Anna, Isabella schedules an appointment with her endocrinologist, who helps Isabella find a diabetic educator to help her manage her blood sugar through her diet. Isabella also starts Virtual Visits online with a mental health professional, who helps her address her stress and anxiety.

Isabella continues scheduling follow-up calls with Anna. Meeting with her clinician every two weeks helps Isabella feel like she’s getting things under control. Thanks to the support, she’s feeling better both mentally and physically.
