Start with a personal wellness plan
When you first access Well onTarget, you are encouraged to complete a brief online Health Assessment. The Health Assessment identifies what you are already doing well and where there may be opportunities for improvement. After you take the assessment, you’ll get a personal wellness report with recommended goals and action steps based on your responses. You also earn Blue PointsSM just for completing the assessment. Get support for your health journey
There are 26 digital, self-management programs available through Well onTarget to support your health journey. When you complete your Health Assessment, Well onTarget uses your responses to recommend which of the courses might benefit you. Each of the programs includes a structured set of activities, support resources as well as clinical and behavioral information specific to your needs.
The variety of subjects available through Well onTarget allow for customization. Some courses, like Quitting Tobacco, Weight Management and Managing Stress, are designed to help you improve your health. Others, like Preventive Health and Sleep Health, are meant to help you maintain a healthy lifestyle.
The programs are either interactive or educational:
- Interactive programs will involve creating daily habits to track, as well as a midpoint and final assessment.
- Educational programs include quizzes, podcasts and other resources embedded within a six-lesson format.
In addition to self-management programs, Well onTarget offers you a variety of other wellness resources including a symptom checker, personalized wellness coaching and health trackers that can sync to most fitness devices.
