Chronic conditions like heart disease, cancer, and diabetes are the leading causes of death and disability in the United States according to the Centers for Disease Control (CDC). Fortunately, most of these diseases can be prevented or controlled by taking care of your health. Participants enrolled in HealthSelect of Texas®, including Consumer Directed HealthSelectSM, have access to benefits and resources that can help manage your health at any stage, at no additional cost.

Health and Wellness

Preventing chronic disease starts with being proactive about your health.

Many chronic diseases are caused by a short list of risky behaviors:

  • Tobacco use and exposure to secondhand smoke
  • Poor nutrition, including diets low in fruits and vegetables and high in sodium and saturated fats
  • Lack of physical activity
  • Excessive alcohol use

If you are struggling in any of these areas, consider enrolling in a Well onTarget® self-management program. If you need more one-on-one help, you can also participate in one of the many coaching programs available through Well onTarget. These resources are available to HealthSelectSM participants at no additional cost. Preventive Care

Your primary care provider (PCP) should be your first stop for preventive care each year. By scheduling annual check-ups with your PCP and getting preventive screenings, you may be able to assess, manage and prevent many health issues. Care Management Program

If you are a HealthSelect participant suffering from a chronic condition, you have extra support for managing various health conditions, including heart disease, diabetes, high blood pressure and asthma, through the Blue Cross and Blue Shield of Texas care management program.

One of the program’s Texas-based clinicians can help you learn about a new diagnosis, understand an upcoming procedure or hospital stay, or help you follow your doctor’s treatment plan. They can also work with your providers to coordinate your treatment plan, arrange appointments and coordinate transportation to and from appointments. Learn more about the care management program.
